- Do you have an eligibility question? Send in a preliminary underwriting opinion or call the IMM Division at Resource Brokerage.
- Send in a complete application within 7 days of signature to avoid a stale date. Make sure all carrier application content is fully legible. (Carriers will not underwrite forms that have been “chopped off” by repeated faxes or if the questions cannot be fully read by the underwriter when it arrives at their desk at time of underwriting).
- Provide full details of any illness or questions answered “Yes”. If possible, provide medical records upfront.
- Advise your client that a telephone interview may be necessary and to respond only to the specific questions asked by the underwriter.
- Complete the physician’s name, address and telephone number.
- Send all necessary forms with the application.
- Sign and date all forms. The applicant’s date is always BEFORE the broker’s date.
- Sign the application and print your name (the producing person) on the back of the application—not the agency’s name—along with the correct tax identification number or your unique id number (UID.)
- Include a copy of your illustration.
- Select a payment plan and submit correct amount of premium with the check made out to the carrier.
