- The new 2-50 Illinois Standard Health Employee Application has been approved and can now be used for all 2-50 Illinois submissions.
- Northwest Indiana 2-50 groups should now utilize the Indiana 2-50 long form application.
- Please note – the current 2-50 Illinois Application will no longer be accepted after January 31, 2011.
- There is no change to the Illinois 51-99 Application. Please remember the UHC 51-99 application only has one medical question.
