JUST A REMINDER!!

Each month, your 2- to 19-lives employer groups insured by BlueCross BlueShield of Illinois (BCBSIL) who are under Illinois State Continuation should be preparing an updated Request for Information (RFI) form listing any additional involuntary terminations or status changes from the previous month.

Now through December 31, 2009, plans subject to COBRA (20+ lives) will receive an updated RFI form from BCBSIL listing new involuntary termination records.  The employer representative should designate with “yes” or “no” which individual is considered eligible for the ARRA COBRA subsidy.

Groups submitting involuntary terminations for the first time can download the RFI from the employer portal at www.bcbsil.com/employer.  The updated RFI should be returned as an attachment using their secure email account, bluecrossblueshield_COBRA@bcbsil.com.  If your employer groups prefer instead to mail or fax the RFI, they may:

• Fax the form to BCBSIL at 618.998.2747 or
• Mail the form to
